Centers Plan for Healthy Living Members: Specialty Care at Home
Your plan may have changed, but your need for consistent eye, foot, and hearing care has not. Aspire brings expert specialty care to former Centers Plan members wherever they live.
For years, Centers Plan for Healthy Living helped members with chronic illnesses and disabilities receive long-term care services in their own homes. In 2025 and 2026, those members were transitioned to new plans, and many families are now sorting out what that means for their care. Aspire Healthcare Solutions helps former Centers Plan for Healthy Living members continue getting the specialty care they need without leaving home. Our team of experts provides mobile podiatry, optometry, audiology, in-home lab collection, PAD testing, and other diagnostic services for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. For members who already rely on care at home, specialty visits that come to the door are a natural fit.

What Happened to Centers Plan for Healthy Living?
Elevance Health, the parent company of Anthem Blue Cross and Blue Shield HP and Wellpoint of NY, acquired Centers Plan on January 1, 2025. As part of that acquisition, Centers Plan members were moved into new plans in two stages:
- Medicaid Advantage Plus (MAP) members: Effective January 1, 2026, members previously enrolled in Centers Plan for Medicaid Advantage Plus were enrolled in Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP), unless they chose a different MAP plan.
- Managed Long-Term Care (MLTC) members: Effective March 1, 2026, members previously enrolled in Centers Plan’s MLTC plan were enrolled in Anthem, or in Wellpoint of NY in certain areas, unless they chose a different MLTC plan.
- Legacy portals: According to Centers Plan, its legacy member and provider portals remain available through February 28, 2027.
If you are not sure which plan you or your loved one has today, check the newest member ID card you received, or call the contact numbers listed on the Centers Plan website for transition help.
Centers Plan for Healthy Living FAQs
Does Centers Plan for Healthy Living still exist?
Centers Plan for Healthy Living no longer operates its member plans the way it once did. Elevance Health, the parent company of Anthem Blue Cross and Blue Shield HP and Wellpoint of NY, acquired Centers Plan on January 1, 2025. Members of the Centers Plan for Medicaid Advantage Plus plan were enrolled in Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP) effective January 1, 2026, and members of the Centers Plan MLTC plan were enrolled in Anthem, or Wellpoint of NY in certain areas, effective March 1, 2026, unless they selected a different plan. Centers Plan states that its legacy member and provider portals remain available through February 28, 2027. If you are unsure which plan you have now, your most recent member ID card or the transition contact numbers on the Centers Plan website can confirm it. Aspire Healthcare Solutions continues to bring specialty care to members who need it at home.
What plan are former Centers Plan MLTC members enrolled in now?
According to Centers Plan, members enrolled in its Managed Long-Term Care plan were transitioned on March 1, 2026. Most members were enrolled in Anthem, while members in certain areas were enrolled in Wellpoint of NY. Members who chose a different MLTC plan before the transition were enrolled in the plan they selected instead. Because each member’s situation may be different, the best way to confirm current coverage is to check the newest ID card received or contact the new plan’s Member Services. Once you know the current plan, the member’s care manager can explain how services such as podiatry, eye care, and hearing care are authorized, and Aspire’s team can help schedule at-home visits.
Do MLTC plans cover podiatry, eye care, and hearing aids?
New York’s managed long-term care benefit package includes podiatry, optometry and eyeglasses, audiology and hearing aids, durable medical equipment, and personal emergency response systems, along with services such as home care, personal care, and care management. These services are generally provided when medically necessary, and many require prior authorization from the plan. For members in partial capitation MLTC plans, Medicare continues to cover doctor visits and lab services. The exact rules for each service, including how often it can be provided, depend on the member’s plan and care plan, so the care manager and Member Handbook are the best sources for details. Aspire Healthcare Solutions provides mobile podiatry, optometry, and audiology at home, helping members access these services without the burden of travel.
What is Anthem HealthPlus Full Dual Advantage LTSS 2?
Anthem HealthPlus Full Dual Advantage LTSS 2 is an HMO Dual-Eligible Special Needs Plan, or D-SNP, offered by Anthem. It is the plan former Centers Plan for Medicaid Advantage Plus members were enrolled in on January 1, 2026, unless they selected a different MAP plan. Plans like this are built for people who qualify for both Medicare and Medicaid and who need long-term services and supports, combining both sets of benefits under one plan to make care easier to coordinate. Covered services, authorization rules, and provider networks are explained in the plan’s Member Handbook and Evidence of Coverage, and Anthem Member Services can answer specific questions. If you or a loved one is enrolled and needs specialty care at home, Aspire Healthcare Solutions can bring eye care, foot care, hearing care, and diagnostic testing to your door.
Can Aspire Healthcare Solutions help me choose or change my plan?
No. Aspire Healthcare Solutions is an independent healthcare provider, not an insurance company, broker, or enrollment counselor. We do not sell, recommend, or enroll members in health plans. If you have questions about choosing or changing a managed long-term care plan, a MAP plan, or a D-SNP, you can contact New York Medicaid Choice, call 1-800-MEDICARE, visit Medicare.gov, or reach out to your State Health Insurance Assistance Program for free, unbiased guidance. The new plan’s Member Services team can also explain your current options. What Aspire does is provide expert at-home specialty care, including podiatry, optometry, audiology, lab collection, and screenings, so members can focus on their health instead of the logistics of getting to appointments.
Why At-Home Specialty Care Fits Former Centers Plan Members
Members of managed long-term care and dual-eligible plans often already receive help at home, such as home care, personal care, or care management. What tends to fall through the cracks is specialty care. A podiatry visit, an eye exam, or a hearing evaluation usually means leaving the house, and for someone who needs daily support, that trip can require a wheelchair van, an aide, and most of the day.
When those appointments become too difficult, they get postponed. Toenails and calluses go untreated, raising the risk of pain and infection for people with diabetes or poor circulation. Glasses prescriptions grow outdated, making reading and moving around the home harder. Hearing loss makes it more difficult to communicate with aides, family members, and care managers. Each of these problems affects safety and quality of life, and each one is easier to manage when the specialist comes to the member instead.
At-Home Services for Centers Plan for Healthy Living Members
- Mobile Podiatry: Foot exams and treatment for diabetic foot care, foot pain, and foot and ankle conditions, plus orthotics.
- Mobile Optometry: Comprehensive eye exams, dilated eye exams, retinal imaging, vision testing, and glasses.
- Diabetic Eye Care: Diabetic retinal exams that help catch eye changes linked to diabetes early.
- Mobile Audiology: Hearing evaluations, hearing aid fittings, and ear wax removal at home.
- PAD Testing: Circulation testing that looks for early signs of peripheral arterial disease.
- Lab Tests at Home: In-home collection for HbA1c, microalbumin, and other lab measures.
- Dental Photography Screening: External oral photography reviewed by a telemedicine dentist.
- Safety and Support: Durable medical equipment, medical alert systems, medical alert key lock boxes, vaccines, and telehealth visits.
How MLTC and Dual-Eligible Benefits Connect to At-Home Care
New York’s managed long-term care benefit package includes many of the services Aspire provides. According to the New York State Department of Health, MLTC plans cover services such as podiatry, optometry and eyeglasses, audiology and hearing aids, durable medical equipment, and personal emergency response systems. For members in partial capitation MLTC plans, Medicare continues to cover doctor visits and lab services, while the MLTC plan covers long-term care services through Medicaid.
Members in MAP plans and D-SNPs, like Anthem HealthPlus Full Dual Advantage LTSS 2, receive Medicare and Medicaid benefits together through one plan. These plans are designed to coordinate both sets of benefits, which can simplify how specialty care is arranged.
Under any of these plans, many services require prior authorization, and plans typically arrange care through a network of participating providers. Your care manager and your plan’s Member Handbook are the best sources for what is covered and how to get it approved.
Aspire Healthcare Solutions is an independent healthcare provider. We do not sell, recommend, or enroll members in health plans. We focus on delivering expert care to the people we serve.
Making At-Home Specialty Care Easier for Families and Care Managers
Coordinating care after a plan change can feel complicated. A little preparation helps visits go smoothly:
- Gather every current card, including the member’s Medicare card, Medicaid card, and new plan ID card
- Ask the member’s care manager whether the needed service requires prior authorization
- Make a short list of concerns, such as foot pain, blurry vision, or trouble hearing
- Note any recent changes in health, medications, or mobility
- Share facility contact information if the member lives in assisted living or a long-term care facility
- Choose a visit time when a family member, aide, or staff member can be present if helpful

The Difference Consistent Specialty Care Makes
For people receiving long-term care at home, small improvements can change daily life in big ways. Regular podiatry care supports more comfortable standing, walking, and transfers. Clear vision makes it easier to recognize faces, read labels, and move safely through the home. Better hearing strengthens communication with aides, family, and care managers, which supports better care overall.
Over time, regular at-home visits also create a clearer health picture. Diabetic retinal exams, PAD testing, and in-home lab work help members and their doctors notice changes early, while consistent foot and eye care helps prevent minor issues from becoming larger ones. When specialty care is easy to receive, it becomes part of the routine instead of something that waits.
