RiverSpring Health Plans Members: Expert Specialty Care at Home
RiverSpring Health Plans now operates as ElderServe Health. Aspire brings podiatry, eye, hearing, and diagnostic care to members wherever they live.
RiverSpring Health Plans has spent decades helping frail older adults and people with disabilities receive the support they need to live as independently as possible. Its members typically face real challenges leaving home, which makes specialty care one of the hardest things to keep up with. Aspire Healthcare Solutions helps solve that problem by bringing specialty care directly to members. Since 2010, our team of experts has provided mobile podiatry, optometry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. Whether your plan materials say RiverSpring or ElderServe, at-home specialty care can help you or your loved one stay on track.

RiverSpring Health Plans Is Now ElderServe Health
If you have noticed new names on plan materials, here is what changed:
- Long history: The organization began in 1996, when the Hebrew Home launched ElderServe to provide managed long-term care plans. It later operated as RiverSpring Health Plans.
- New affiliation: The New York Department of Health approved an affiliation between CareSource, a national nonprofit managed care organization, and ElderServe Health effective September 2025.
- Separate from RiverSpring Living: As part of that transition, RiverSpring Living, the organization’s former parent, ended its affiliation with the health plan and continues to focus on residential, supportive, and assisted living.
- Plans under the ElderServe name: The health plan now presents its plans as ElderServe at Home (MLTC), ElderServe MAP (HMO D-SNP), and ElderServe Star (HMO I-SNP).
- Coverage continuity: CareSource stated that no disruption to member coverage was anticipated as ElderServe Health joined its family of companies.
If you are unsure which plan you or a loved one has, the most recent member ID card or the plan’s Member Services line can confirm it.
RiverSpring Health Plans FAQs
What happened to RiverSpring Health Plans?
RiverSpring Health Plans was the business name of ElderServe Health, Inc., a not-for-profit managed care organization serving frail older adults and people with disabilities. In March 2025, CareSource and ElderServe Health announced plans to affiliate, and the New York Department of Health approved the affiliation effective September 2025. As part of the change, RiverSpring Living ended its affiliation with the health plan, and the plan now presents its products under the ElderServe name: ElderServe at Home, ElderServe MAP, and ElderServe Star. CareSource stated that no disruption to member coverage was anticipated during the transition. If you have questions about your current coverage, your most recent member materials and Member Services are the best sources. Aspire Healthcare Solutions continues to bring specialty care to members at home.
Is RiverSpring Health Plans the same as RiverSpring Living?
Not anymore. RiverSpring Living, which began as the Hebrew Home in 1917, launched the health plan in 1996 under the name ElderServe, and the two organizations were affiliated for many years. With the CareSource affiliation approved in 2025, RiverSpring Living ended its affiliation with the health plan. RiverSpring Living now focuses on residential, supportive, and assisted living services, while the health plan operates as ElderServe Health within the CareSource family. Because the names are similar, many people still search for one when they mean the other. If you need help with your health plan coverage, contact ElderServe Health Member Services. If you need specialty care at home, Aspire Healthcare Solutions can help.
What is ElderServe at Home?
ElderServe at Home is a New York State managed long-term care program, previously associated with the RiverSpring name. It is designed to help members remain in their own homes and live as independently as possible. The plan works with each member’s physician to provide services tailored to the member’s needs, monitors both medical and social needs, and updates the plan of care as conditions change. New York’s managed long-term care benefit package includes services such as home care, personal care, podiatry, optometry and eyeglasses, audiology and hearing aids, and durable medical equipment, typically arranged through the member’s care plan. Because ElderServe at Home members often find travel difficult, at-home specialty visits from Aspire Healthcare Solutions can make foot, eye, and hearing care far easier to keep up with.
What is ElderServe Star?
ElderServe Star (HMO I-SNP), previously listed as RiverSpring Star, is an Institutional Special Needs Plan. These Medicare Advantage plans are designed for people who live in an institutional setting, such as a nursing home, or who need that level of care. Institutional Special Needs Plans focus on the needs of residents with complex health conditions and often emphasize care coordination within the facility. As a Medicare Advantage plan, ElderServe Star covers the medically necessary services Original Medicare covers, and its 2026 plan listing shows dental, vision, and hearing benefits. Because residents in institutional settings face significant barriers to outside appointments, on-site specialty care from Aspire Healthcare Solutions can help them receive eye, foot, hearing, and diagnostic care without leaving the building.
What is ElderServe MAP?
ElderServe MAP (HMO D-SNP), previously listed as RiverSpring MAP, is a Medicaid Advantage Plus plan for people who qualify for both Medicare and Medicaid and need long-term care services. MAP plans bring Medicare Advantage coverage and Medicaid long-term care benefits together in one plan, so medical care and long-term support can be coordinated more easily. As a Medicare Advantage plan, ElderServe MAP covers the medically necessary services Original Medicare covers, and its 2026 plan listing shows dental, vision, and hearing benefits. Long-term care services, such as home care, are covered through the Medicaid side of the plan. For ElderServe MAP members who find travel difficult, Aspire Healthcare Solutions brings specialty care such as podiatry, eye exams, hearing care, and lab collection directly to the home.
Three Plans, One Common Thread: Care Where Members Live
ElderServe at Home (MLTC) is a New York State managed long-term care program designed to help members remain in their own homes and live as independently as possible. The plan works with each member’s physician and adjusts the plan of care as needs change. Because members already receive support at home, specialty visits at home keep their care in one familiar place.
ElderServe MAP (HMO D-SNP) coordinates Medicare and Medicaid benefits for dual-eligible members, bringing medical care and long-term care support together in one plan.
ElderServe Star (HMO I-SNP) is an Institutional Special Needs Plan designed for Medicare beneficiaries who live in an institutional setting or need that level of care. For these members, on-site specialty care avoids the strain of outside appointments.
The Specialty Services Members Need Most, Delivered at Home
- Mobile Podiatry: Diabetic foot care, treatment for foot pain and foot and ankle conditions, and orthotics
- Mobile Optometry: Comprehensive, dilated, and medical eye exams, retinal imaging, vision testing, and glasses
- Diabetic Eye Care: Diabetic retinal exams that help catch eye changes early
- Mobile Audiology: Hearing evaluations, hearing aid fittings, and ear wax removal
- PAD Testing: Circulation testing for early signs of peripheral arterial disease
- Lab Collection: In-home HbA1c, microalbumin, and other lab measures
- Dental Photography Screening: External oral photos reviewed by a telemedicine dentist
- Safety and Support: Durable medical equipment, medical alert systems, medical alert key lock boxes, vaccines, and telehealth
Why Frail and Homebound Members Miss Specialty Care
Members of plans like ElderServe at Home, MAP, and Star often need help with daily activities, use wheelchairs or walkers, or live with conditions such as dementia, heart disease, or diabetes. For them, a specialist visit can require accessible transportation, an aide or family member to accompany them, and hours of travel and waiting. Many simply cannot make the trip without significant strain.
When specialty care is delayed, the effects show up in daily life. Untreated foot problems can make standing and transfers painful. Outdated glasses increase the difficulty of reading, eating, and moving safely. Hearing loss can make it harder for members to communicate with aides, care managers, and loved ones. At-home specialty care addresses these needs promptly and comfortably, without disrupting the support already in place.
How Plan Benefits Connect to At-Home Specialty Care
For ElderServe at Home members, New York’s managed long-term care benefit package includes services such as podiatry, optometry and eyeglasses, audiology and hearing aids, durable medical equipment, and personal emergency response systems. These services are typically arranged through the member’s plan of care and may require authorization.
ElderServe MAP and ElderServe Star are Medicare Advantage plans, so they cover the medically necessary services Original Medicare covers. Under Medicare Part B, that includes a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, diagnostic hearing and balance exams, and medically necessary lab tests. Plan listings for 2026 show dental, vision, and hearing benefits on both plans, with details described in each plan’s Evidence of Coverage.
The member’s care manager and plan documents are the best sources for what is covered and how to arrange it. Aspire Healthcare Solutions is an independent healthcare provider. We do not sell, recommend, or enroll members in health plans. Our focus is bringing expert care home.
