Fidelis Care Providers for At-Home Healthcare

Expert eye, foot, hearing, and diagnostic care for Fidelis Care and Wellcare by Fidelis Care members, brought to your home or care community.

Fidelis Care serves more than 1.7 million New Yorkers, making it one of the largest health plans in the state. With that many members, searching for a Fidelis Care provider is one of the most common first steps toward getting care. For many seniors and people with chronic conditions, though, the harder step comes next: actually getting to the appointment. Aspire Healthcare Solutions removes that step by bringing specialty care directly to Fidelis Care and Wellcare by Fidelis Care members. Since 2010, our team of experts has provided mobile optometry, podiatry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities.

Fidelis Care At-Home Care FAQs

  • What is Wellcare by Fidelis Care?

Wellcare by Fidelis Care is the name of Fidelis Care’s Medicare Advantage plans. Fidelis Care is part of Centene, which also owns the Wellcare brand, so its Medicare plans carry both names. For 2026, Wellcare by Fidelis Care plans include Wellcare Fidelis Dual Align (HMO D-SNP) and Wellcare Fidelis Dual Liberty Sync (HMO D-SNP) for members with both Medicare and Medicaid, along with Wellcare Fidelis Patriot Simple (HMO-POS). These plans combine medical and prescription drug coverage, with extra benefits that vary by plan. Your Summary of Benefits or Evidence of Coverage explains exactly what your plan includes. If you are a Wellcare by Fidelis Care member who needs specialty care but finds it hard to travel, Aspire Healthcare Solutions can bring eye care, foot care, hearing care, and lab testing to your home.

  • What happened to Wellcare Fidelis Dual Access and Dual Plus?

Fidelis Care made two important changes to its dual-eligible plans for 2026. Members enrolled in Wellcare Fidelis Dual Access (HMO D-SNP) were moved into Wellcare Fidelis Dual Liberty Sync (HMO D-SNP) effective January 1, 2026. Separately, Wellcare Fidelis Dual Plus (HMO D-SNP) was renamed Wellcare Fidelis Dual Align (HMO D-SNP) on the same date. If you or a loved one had either of these plans, your Annual Notice of Change and your newest member ID card explain your current coverage, and Fidelis Care Member Services can answer questions about what changed. Medicare Advantage plans can change their benefits every year, so reviewing your plan materials each fall is always a good idea. Aspire continues to provide at-home specialty care for members who need it.

  • Does Fidelis Care cover eye exams and glasses?

Vision coverage depends on which Fidelis Care plan you have. Wellcare by Fidelis Care Medicare Advantage plans cover the eye care Original Medicare covers, including a yearly diabetic retinopathy exam for people with diabetes and medically necessary exams for eye conditions, and many plans add routine vision benefits. For Fidelis Care at Home members, optometry and eyeglasses are part of New York’s managed long-term care benefit package. Medicaid Managed Care, Child Health Plus, and Essential Plan members have their own vision benefits described in the Member Handbook. Frequency limits and eyewear rules vary, so confirming your benefits before a visit is the best approach. Aspire Healthcare Solutions provides mobile optometry at home, including comprehensive and dilated eye exams, retinal imaging, vision testing, diabetic eye care, and glasses.

  • What is Fidelis Care at Home?

Fidelis Care at Home is Fidelis Care’s managed long-term care plan. Managed long-term care plans in New York serve people who need ongoing help to live safely in the community, coordinating services such as home care and personal care through a care plan developed with the member. New York’s managed long-term care benefit package also includes services like podiatry, optometry and eyeglasses, audiology and hearing aids, and durable medical equipment, which are typically arranged and authorized through the member’s care manager. Because many Fidelis Care at Home members already receive support at home and find travel difficult, at-home specialty visits from Aspire Healthcare Solutions can make it much easier to keep up with foot, eye, and hearing care.

  • Does Fidelis Care cover at-home medical visits?

Coverage for an at-home visit depends on your Fidelis Care plan, the service you receive, and whether that service is medically necessary. Wellcare by Fidelis Care Medicare Advantage plans cover the medically necessary services Original Medicare covers, including several services Aspire provides at home, such as diabetic eye exams, diabetic foot care, diagnostic hearing exams, and medically necessary lab tests. Fidelis Care at Home, Medicaid Managed Care, and other Fidelis Care plans each have their own covered services and authorization rules. Some services may need a referral or prior approval. Reviewing your Evidence of Coverage or Member Handbook, or calling the Member Services number on your ID card, is the best way to confirm coverage before your visit. Once you know what your plan includes, Aspire’s team can bring care directly to your home.

Finding a Fidelis Care Doctor Is Step One. Getting There Is Step Two.

A provider directory can tell you who participates in your plan. It cannot tell you how you will get to the office, how long you will wait, or how you will feel after a long day of travel. For members who use a walker or wheelchair, rely on family for rides, or live with chronic pain or fatigue, that second step is often where care breaks down.

At-home specialty care solves the second step entirely. Instead of arranging transportation and spending a day at an appointment, members receive eye exams, foot care, hearing evaluations, and lab collection in the comfort of home. Visits are easier to schedule, easier to keep, and far less disruptive for both patients and caregivers. Before your visit, it is always smart to confirm coverage for your specific plan through Fidelis Care’s provider directory or Member Services.

How Wellcare by Fidelis Care Medicare Plans Work With At-Home Care

Wellcare by Fidelis Care offers several Medicare Advantage plans. For 2026, these include Wellcare Fidelis Dual Align (HMO D-SNP), Wellcare Fidelis Dual Liberty Sync (HMO D-SNP), and the new Wellcare Fidelis Patriot Simple (HMO-POS). The Dual plans are built for members who have both Medicare and Medicaid and combine prescription drug coverage with medical benefits in one plan.

Every Wellcare by Fidelis Care Medicare Advantage plan covers the medically necessary services Original Medicare covers. Under Medicare Part B, that includes a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, diagnostic hearing and balance exams, and medically necessary lab tests. Many plans add extra benefits beyond Original Medicare, and those vary by plan. Some services may require a referral or prior authorization.

For Fidelis Care at Home members, New York’s managed long-term care benefit package includes services such as podiatry, optometry and eyeglasses, audiology and hearing aids, and durable medical equipment, usually arranged through the member’s care plan.

Your Evidence of Coverage, Member Handbook, or care manager is the best source for your plan’s specific benefits. Aspire Healthcare Solutions is an independent healthcare provider. We do not sell insurance, recommend plans, or enroll members. We focus on bringing expert care home.

A Closer Look at Diabetes Care at Home

Diabetes affects nearly every part of the body, and some of its most serious complications develop quietly. That is why people living with diabetes are usually advised to keep up with several types of care each year. At-home visits make it possible to handle many of them without leaving the house:

  • Eyes: Diabetic retinal exams look for changes in the retina that may not cause symptoms early on.
  • Feet: Podiatry visits check for nerve changes, circulation problems, skin breakdown, and nail or callus issues that can lead to complications.
  • Circulation: PAD testing helps identify reduced blood flow in the legs, which is more common in people with diabetes.
  • Labs: HbA1c testing shows how blood sugar has been trending, and microalbumin testing helps monitor kidney health.

When these visits happen at home, members and their doctors get a clearer, more consistent picture of how diabetes is being managed over time.
