VillageCareMAX Members: Eye, Foot, and Hearing Care at Home
Expert specialty care for VillageCareMAX MLTC, Medicaid Advantage Plus, and Medicare Advantage members, delivered to your home or care community.
VillageCareMAX is built around helping members live safely and independently at home, with care coordination that supports their quality of life. Aspire Healthcare Solutions extends that same idea to specialty care. Instead of traveling to separate offices for foot care, eye exams, and hearing evaluations, VillageCareMAX members can receive those services where they live. Since 2010, our team of experts has provided mobile podiatry, optometry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. For members managing chronic conditions or receiving long-term care at home, specialty visits that come to the door make consistent care far more achievable.

VillageCareMAX At-Home Care FAQs
What is VillageCareMAX?
VillageCareMAX is a health plan that is part of VillageCare, an organization that has provided care to older adults and people with chronic diseases and conditions for more than 45 years. VillageCareMAX helps Medicaid and Medicare eligible individuals live safely and independently at home through managed long-term care and Medicare Advantage plans, with care coordination that supports quality of life. Its plans include a Managed Long-Term Care (MLTC) Plan, the Medicare Total Advantage Plan (HMO D-SNP), the Medicare Health Advantage Plan (HMO D-SNP), and the Medicare Select Advantage Plan (HMO). Because many members find travel difficult, Aspire Healthcare Solutions complements VillageCareMAX’s support by bringing eye care, foot care, hearing care, and lab collection directly to members at home.
What is the VillageCareMAX Medicare Total Advantage Plan?
The VillageCareMAX Medicare Total Advantage Plan (HMO D-SNP) is a Medicaid Advantage Plus plan for people who have both Medicare and Medicaid and need community-based long-term care services. It covers Medicare benefits together with Medicaid long-term care services in one plan, and VillageCareMAX lists extra benefits such as dental, vision, and hearing coverage, routine transportation, acupuncture, a fitness membership, and an over-the-counter benefit. Some supplemental benefits are available only to members who meet specific chronic condition criteria. Cost sharing can vary based on the member’s level of Medicaid and Extra Help. The plan’s materials explain all benefits in detail. Aspire Healthcare Solutions brings specialty care to Medicare Total Advantage members at home.
Does VillageCareMAX MLTC cover eye care and foot care?
Yes. VillageCareMAX lists dental, eye, and foot care among the long-term care benefits covered by its Managed Long-Term Care Plan, along with in-home care, medical supplies and equipment, a personal emergency response system, adult day health care, and consumer directed personal assistance. As with any managed long-term care plan, these services are generally provided when medically necessary, are usually arranged through the member’s care plan, and may require prior authorization. The member’s care manager and plan materials are the best sources for how each service is approved. Aspire Healthcare Solutions provides mobile optometry and podiatry at home, including eye exams, retinal imaging, glasses, diabetic foot care, treatment for foot pain, and orthotics.
What is the difference between the VillageCareMAX Medicare Health Advantage and Medicare Select Advantage plans?
The Medicare Health Advantage Plan (HMO D-SNP) is a Special Needs Plan for members who have both Medicare and Medicaid, offering services and extra benefits at little or no cost, including dental, vision, and hearing coverage and routine transportation. The Medicare Select Advantage Plan (HMO) is a Medicare Advantage prescription drug plan for members who have Medicare and are eligible for Extra Help. It includes low-cost prescription drug coverage, dental, vision, and hearing coverage, diabetic supplies, and an over-the-counter benefit. Both plans are HMO plans that generally use a network of participating providers, and some services may require a referral or prior authorization. Your plan materials explain your specific benefits. Aspire Healthcare Solutions provides at-home specialty care for members of either plan.
Does VillageCareMAX cover hearing aids?
VillageCareMAX’s Medicare plans include hearing coverage, and the Medicare Select Advantage Plan’s over-the-counter benefit lists over-the-counter hearing aids among the eligible items. For members of the Managed Long-Term Care Plan, audiology and hearing aids are part of New York’s managed long-term care benefit package. The details, including which providers can be used, how often hearing aids are covered, and whether prior authorization is needed, depend on the specific plan. Reviewing your plan materials or calling VillageCareMAX Member Services is the best way to confirm your benefits. Aspire Healthcare Solutions offers at-home hearing evaluations, hearing aid fittings, and ear wax removal, giving members a comfortable way to address hearing loss without leaving home.
Where VillageCareMAX Benefits Meet Aspire’s At-Home Specialty Care
Several benefits VillageCareMAX highlights line up closely with the services Aspire brings to the home:
- Eye care: The MLTC plan lists eye care among its covered benefits, and the Medicare plans include vision coverage. Aspire provides comprehensive and dilated eye exams, retinal imaging, vision testing, diabetic retinal exams, and glasses at home.
- Foot care: The MLTC plan lists foot care among its covered benefits. Aspire’s mobile podiatry team provides diabetic foot care, treatment for foot pain and foot and ankle conditions, and orthotics.
- Hearing care: VillageCareMAX’s Medicare plans include hearing coverage. Aspire offers at-home hearing evaluations, hearing aid fittings, and ear wax removal.
- Medical supplies and equipment: The MLTC plan covers medical supplies and equipment. Aspire provides durable medical equipment to support health, mobility, and daily living.
- Personal emergency response system: The MLTC plan covers PERS. Aspire offers medical alert systems and medical alert key lock boxes.
Covered services generally must be medically necessary, may require prior authorization, and are typically provided through the plan’s network. Your care manager and plan materials are the best sources for how a specific service is covered. Aspire Healthcare Solutions is an independent healthcare provider. We do not sell, recommend, or enroll members in health plans.

Why a Ride Is Not Always Enough
Several VillageCareMAX plans include routine transportation, which helps many members reach their appointments. Still, a ride only covers the distance. It does not remove the effort of getting dressed and ready, transferring in and out of a vehicle, navigating a building, waiting to be seen, and recovering afterward. For members who use wheelchairs, live with chronic pain or fatigue, or rely on aides for daily activities, that effort can make an appointment feel impossible.
At-home specialty care takes the trip out of the equation entirely. The visit happens in a familiar setting, aides and family members can be present, and the member can rest as soon as it is over. Transportation benefits can then be saved for appointments that truly require an office or hospital.
Living With a Chronic Condition? Specialty Care at Home Can Help
Many VillageCareMAX members manage chronic conditions such as diabetes, heart disease, or the effects of a stroke. These conditions often call for regular specialty care, and at-home visits make that care easier to keep up with:
- Diabetes: Diabetic retinal exams, podiatry visits for foot checks and care, and in-home HbA1c and microalbumin lab collection.
- Heart and circulation concerns: PAD testing to check blood flow in the legs, supported by Aspire’s experience with cardiovascular risks and atherosclerosis.
- Limited mobility after illness or stroke: Eye, foot, and hearing care without the physical strain of travel.
- Ongoing safety at home: Medical alert systems and key lock boxes that help members and families feel prepared between visits.
Regular at-home specialty visits give members and their doctors a clearer picture of health over time, supporting earlier conversations when something changes.
