Cigna Medicare Providers for At-Home Healthcare
Cigna Medicare is now HealthSpring. Aspire brings expert eye, foot, hearing, and diagnostic care to True Choice PPO members at home.
Cigna Medicare members have always valued choice, and that is exactly what the True Choice PPO name was built around. But flexibility on paper does not help much when getting to an appointment is the hardest part. Aspire Healthcare Solutions makes specialty care practical for Cigna Medicare members by bringing it home. Since 2010, our team of experts has provided mobile optometry, podiatry, audiology, in-home lab work, PAD testing, and diagnostic screenings for seniors in private homes, senior apartments, assisted living communities, and long-term care facilities. Whether your card says Cigna or HealthSpring, our goal is the same: expert care at your door, on a schedule that respects your day.

Cigna Medicare Is Now HealthSpring: What Changed and What Did Not
- What changed: On March 19, 2025, The Cigna Group completed the sale of its Medicare Advantage, Medicare prescription drug, and Medicare supplemental benefits businesses to Health Care Service Corporation (HCSC). HCSC then renamed those Medicare products HealthSpring.
- What stayed familiar: The True Choice plan name carried over. For 2026, HealthSpring offers True Choice (PPO) Medicare Advantage plans, including plans available across the five New York City boroughs.
- What Cigna still offers: Cigna Healthcare continues to provide employer and individual health coverage. Its Medicare Advantage, Part D, and Medicare Supplement products now belong to HealthSpring.
- Where to get answers: If you are unsure which company manages your Medicare coverage today, the Member Services number on your most recent ID card can confirm it.
Many members still search for “Cigna Medicare” or “Cigna True Choice,” and that is perfectly normal. Aspire provides at-home care for members under either name.
Cigna Medicare At-Home Care FAQs
Is Cigna Medicare now HealthSpring?
Yes. The Cigna Group completed the sale of its Medicare Advantage, Medicare prescription drug, and Medicare supplemental benefits businesses to Health Care Service Corporation, also known as HCSC, on March 19, 2025. HCSC later renamed those Medicare products HealthSpring, and 2026 plans are offered under the HealthSpring name. Cigna Healthcare continues to offer employer and individual health coverage, but its former Medicare Advantage and Medicare Supplement plans are now managed by HealthSpring. If you previously had Cigna Medicare coverage and are unsure what changed, your Annual Notice of Change or the Member Services number on your most recent ID card can explain your current plan. Aspire Healthcare Solutions provides at-home specialty care for members who know their plan as Cigna Medicare or HealthSpring.
Is Cigna True Choice Medicare PPO still available?
The True Choice name continues under HealthSpring. For 2026, HealthSpring offers True Choice (PPO) Medicare Advantage plans, including plans available throughout the five New York City boroughs. These plans combine Medicare Part A and Part B coverage with prescription drug coverage and use a PPO network that gives members flexibility to see providers in and out of network. Medicare Advantage plans can change their benefits, costs, and availability every year, so reviewing your Annual Notice of Change each fall is the best way to see what is changing for the coming year. If you are a True Choice PPO member who needs eye care, foot care, hearing care, or lab work but finds it hard to travel, Aspire Healthcare Solutions can bring that care to your home.
Can I see out-of-network providers with a Cigna or HealthSpring True Choice PPO plan?
PPO plans like True Choice generally allow members to see providers outside the plan’s network, which is one of the main reasons people choose them. The tradeoff is cost. Out-of-network care usually comes with higher cost sharing than in-network care, and some PPO plans have separate out-of-pocket limits for in-network and out-of-network services. Certain services may also require prior authorization regardless of where you receive them. Your Summary of Benefits and Evidence of Coverage explain how your specific plan handles out-of-network care, and HealthSpring Member Services can confirm costs for a particular service. Knowing these details ahead of time helps you plan at-home specialty visits with confidence.
Does Cigna Medicare cover podiatry and foot care?
Cigna Medicare plans, now offered as HealthSpring, cover the podiatry services Original Medicare covers, and HealthSpring’s 2026 True Choice plan documents list Medicare-covered podiatry as a benefit. Under Original Medicare rules, that includes foot exams and treatment for people with diabetes-related nerve damage, along with medically necessary care for injuries and conditions of the foot and ankle. Routine foot care, such as basic nail trimming for people without a qualifying medical condition, is generally not covered by Original Medicare, although some Medicare Advantage plans offer additional foot care benefits. Your plan documents explain what your specific plan includes. Aspire Healthcare Solutions offers mobile podiatry at home, including diabetic foot care, treatment for foot pain, and orthotics, so members can protect their foot health without traveling to an office.
Do I need a referral to see a specialist with Cigna Medicare True Choice PPO?
PPO plans like True Choice generally do not require a referral from a primary care provider before seeing a specialist, which can make it easier to schedule services such as eye exams, podiatry, and hearing evaluations. However, some services may still require prior authorization from the plan, and cost sharing can vary depending on whether the provider is in or out of network. Because rules differ from plan to plan and can change each year, checking your Evidence of Coverage or calling HealthSpring Member Services before your visit is the best way to avoid surprises. Once you know your plan’s requirements, Aspire’s team can help you schedule an at-home visit at a time that works for you.
What True Choice PPO Flexibility Means for At-Home Specialty Care
A PPO, or Preferred Provider Organization, gives Medicare Advantage members more freedom than an HMO. True Choice PPO members can generally see providers both in and out of the plan’s network, and they usually do not need a referral to see a specialist. Out-of-network care typically costs more than in-network care, so understanding how your plan handles both is worth a few minutes before scheduling any visit.
HealthSpring’s 2026 True Choice plan documents describe coverage for Medicare-covered eye exams, eyewear, hearing exams, and podiatry, among many other services. Under Original Medicare rules, that includes a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, diagnostic hearing and balance exams, and medically necessary lab tests. Many Medicare Advantage plans also add routine vision, hearing, or dental benefits, and the specifics vary by plan.
Your Summary of Benefits and Evidence of Coverage list exactly what your plan includes, what you pay in and out of network, and which services need prior authorization. HealthSpring Member Services can answer any questions.
Aspire Healthcare Solutions is an independent healthcare provider. We do not sell insurance, recommend plans, or handle enrollment. Our focus is bringing expert care to your home.
Five Ways Aspire Brings Care Home for Cigna Medicare Members
- Eye care that does not require a drive. Comprehensive eye exams, dilated eye exams, retinal imaging, vision testing, and glasses, along with diabetic retinal exams that help catch eye changes early.
- Foot care that supports independence. Mobile podiatry for diabetic foot care, foot pain, and foot and ankle conditions, plus orthotics for comfort and stability.
- Hearing care in a comfortable setting. At-home hearing evaluations, hearing aid fittings, and professional ear wax removal.
- Testing without the extra errand. PAD testing for circulation and in-home lab collection for HbA1c, microalbumin, and other important measures.
- Support for safer daily living. Medical alert systems, medical alert key lock boxes, durable medical equipment, vaccines, telehealth visits, and dental photography screenings reviewed by a telemedicine dentist.
Signs It May Be Time to Schedule an At-Home Visit
Specialty care is often delayed because the warning signs feel minor. If you or a loved one notices any of these, an at-home visit can be a simple next step:
- Reading, watching TV, or recognizing faces has become harder
- It has been more than a year since a diabetic eye exam
- Feet feel numb, tingly, or painful, or small cuts are slow to heal
- Toenails or calluses have become difficult to manage at home
- Legs ache or cramp while walking and ease with rest
- The television volume keeps creeping up, or conversations are hard to follow
- Lab work ordered by a doctor has not been completed
- Getting to appointments has become stressful, tiring, or unsafe
Who Benefits Most From At-Home Care With Cigna Medicare
At-home specialty care is especially valuable for Cigna Medicare and HealthSpring members whose care needs are ongoing but whose ability to travel has changed. Seniors managing diabetes often need eye exams, foot care, and lab work every year, and receiving them at home makes it far easier to stay on schedule. Members who use canes, walkers, or wheelchairs, or who are recovering from surgery or illness, can avoid the physical strain of getting to an office.
Residents of assisted living communities and long-term care facilities benefit from on-site visits that do not require arranging transportation or staff support. And for the adult children and spouses who coordinate care, at-home visits mean fewer missed workdays and far less worry about how a loved one will get to the next appointment.
Why Cigna Medicare Members Choose Aspire
- A recognized name in at-home care since 2010: Aspire started with customized in-home eye care and has grown into a full range of mobile specialty services.
- Experts across specialties: Our team includes nurse practitioners, vascular specialists, cardiologists, podiatrists, audiologists, and other skilled professionals.
- One provider, many needs: Eye, foot, hearing, lab, and screening services are available through a single team.
- Strength in early detection: We focus on identifying cardiovascular risks, atherosclerosis, and diabetes-related complications early.
- Care in any setting: We serve private homes, senior apartments, assisted living communities, and long-term care facilities.
- Scheduling that fits your life: We adapt to your routine instead of asking you to adapt to ours.
- Award-recognized, compassionate service: Every visit centers on the comfort, dignity, and individual needs of the patient.
Care That Keeps Paying Off
The benefits of an at-home visit extend well beyond the appointment itself. Updated glasses can make everyday tasks like reading mail and managing medications easier. Hearing care helps members stay engaged in conversations with family and friends. Consistent foot care supports comfortable walking and an active routine.
Over time, at-home care builds a habit of prevention. When appointments are easy to keep, members keep them, and regular retinal exams, PAD testing, and lab work give members and their doctors a clearer picture of health from year to year. That information supports earlier conversations about care whenever something changes.
