Aspire Healthcare Solutions meets that challenge by bringing the specialist to you. Since 2010, our team of experts has provided mobile podiatry, optometry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. Whether you have a Healthfirst Medicare Advantage plan, a dual-eligible plan, or a long-term care plan like CompleteCare or Senior Health Partners, at-home visits make it easier to stay on top of the care you need.

Healthfirst At-Home Care FAQs
How do I find a doctor that takes Healthfirst?
The most reliable way to find providers who participate in your Healthfirst plan is to use the Find a Doctor tool on Healthfirst’s website or call the Member Services number on your ID card. Healthfirst networks can differ by plan, and provider participation can change, so checking your specific plan before scheduling helps you avoid unexpected costs. Healthfirst notes that out-of-network providers are under no obligation to treat its Medicare members except in emergencies, and out-of-network care may cost more. If you are looking for a specialist such as a podiatrist, eye doctor, or audiologist and find it difficult to travel, Aspire Healthcare Solutions brings those services directly to your home. Call our team to discuss your needs and your plan.
Does Healthfirst Medicare cover podiatry?
Healthfirst Medicare Advantage plans cover the podiatry services Original Medicare covers. That includes foot exams and treatment for people with diabetes-related nerve damage, along with medically necessary care for injuries and conditions of the foot and ankle. Routine foot care, such as basic nail trimming for people without a qualifying medical condition, is generally not covered under Original Medicare rules, although some plans include additional foot care benefits. For members of Senior Health Partners or CompleteCare, podiatry is part of New York’s long-term care benefit package and is usually arranged through the member’s care team. Your Evidence of Coverage explains exactly what your plan includes. Aspire Healthcare Solutions offers mobile podiatry at home, including diabetic foot care, treatment for foot pain, and orthotics.
Does Healthfirst cover eye exams and glasses?
Vision coverage depends on your Healthfirst plan. All Healthfirst Medicare Advantage plans cover the eye care Original Medicare covers, including a yearly diabetic retinopathy exam for people with diabetes and medically necessary exams for eye conditions. Many Healthfirst Medicare plans also include vision coverage that can apply to routine eye exams and eyewear, and Healthfirst notes that its eyewear allowance can only be used at participating retailers. Senior Health Partners lists vision services among its benefits, and Healthfirst’s Medicaid and Essential Plan members have their own vision benefits described in their Member Handbook. Aspire Healthcare Solutions provides mobile optometry at home, including comprehensive and dilated eye exams, retinal imaging, vision testing, diabetic eye care, and glasses.
What is Healthfirst CompleteCare?
Healthfirst CompleteCare (HMO D-SNP) is an all-in-one Medicaid Advantage Plus plan for people who have Medicare and full Medicaid and need long-term care services. It combines medical coverage, prescription drug coverage, and long-term care services and supports, such as personal care aides and adult day health care, in a single plan. Members have a dedicated care team that helps them meet their care goals and live independently at home, and Healthfirst offers caregiver support through Carallel. Eligibility includes being 18 or older, living in the plan’s service area, having Original Medicare and full Medicaid, and needing community-based long-term care services for more than 120 days, among other requirements. Because many CompleteCare members already receive care at home, Aspire’s at-home specialty visits fit naturally into their routine.
What is Senior Health Partners?
Senior Health Partners is Healthfirst’s Managed Long-Term Care Medicaid plan. It provides and coordinates long-term care services such as home care, personal care, adult day health care, home-delivered meals, and skilled nursing home care, and it lists a personal emergency response system and hearing, dental, and vision services among its benefits. Members have a care management team that helps coordinate services. Eligibility includes being 18 or older, having Medicaid, living in the plan’s service area, and being assessed as needing community-based long-term care services for more than 120 days, along with other requirements. Senior Health Partners members often find travel especially difficult, and Aspire Healthcare Solutions can bring podiatry, eye care, hearing care, and lab collection directly to their homes.
Searching for a Healthfirst Podiatrist or Eye Doctor?
Foot care and eye care are two of the specialties Healthfirst members search for most, and for good reason. For older adults and people with diabetes, regular podiatry visits help prevent painful and potentially serious foot problems, and regular eye exams help catch conditions like diabetic retinopathy before they affect daily life.
Aspire’s mobile podiatry team treats diabetic foot concerns, foot pain, and foot and ankle conditions at home and provides orthotics. Our mobile optometry team performs comprehensive eye exams, dilated eye exams, retinal imaging, vision tests, and diabetic retinal exams, and helps with glasses. Before scheduling, it is always wise to confirm coverage for your specific plan using Healthfirst’s Find a Doctor tool or by calling Member Services.
Specialty Care Healthfirst Members Can Receive at Home
- Podiatry: Diabetic foot care, treatment for foot pain and foot and ankle conditions, and orthotics
- Optometry: Comprehensive, dilated, and medical eye exams, retinal imaging, vision testing, and glasses
- Diabetic eye care: Diabetic retinal exams that help identify eye changes early
- Audiology: Hearing evaluations, hearing aid fittings, and ear wax removal
- PAD testing: Circulation testing for early signs of peripheral arterial disease
- Lab tests at home: In-home collection for HbA1c, microalbumin, and other lab measures
- Dental photography screening: External oral photos reviewed by a telemedicine dentist
- Home safety and support: Medical alert systems, medical alert key lock boxes, durable medical equipment, vaccines, and telehealth visits
Rides Help. Home Visits Help More.
Many Healthfirst plans include transportation benefits, with one-way rides to in-network providers. That is a valuable benefit, and for some members it makes all the difference. But a ride solves only part of the problem. Members still have to get ready, get to the curb, transfer in and out of a vehicle, navigate a building, wait to be seen, and make the trip home. For someone who uses a wheelchair, lives with chronic pain, or tires easily, that sequence can turn a 20-minute appointment into an exhausting day.
At-home specialty care removes every one of those steps. The clinician arrives with the equipment needed for the visit, the appointment happens in a familiar setting, and the member can rest the moment it is over. Rides can then be saved for the appointments that truly require an office or hospital visit.
How Healthfirst Coverage Works With At-Home Specialty Care
Most Healthfirst Medicare Advantage plans are HMO plans that use a network of participating providers, and Healthfirst’s Signature (PPO) plan allows members to see out-of-network providers who accept Medicare, usually at a higher cost. Some services may require a referral or prior authorization depending on the plan.
Every Healthfirst Medicare Advantage plan covers the medically necessary services Original Medicare covers. Under Medicare Part B, that includes a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, diagnostic hearing and balance exams, and medically necessary lab tests. Healthfirst plans also include extra benefits, such as vision and hearing coverage on many plans, and these vary from plan to plan.
For long-term care members, Senior Health Partners lists hearing, dental, and vision services and a personal emergency response system among its plan highlights, and CompleteCare combines Medicare coverage with long-term care services and supports. These services are typically arranged through the member’s care team.
