Elderplan Providers for At-Home Specialty Care
Expert eye, foot, hearing, and diagnostic care for Elderplan and HomeFirst members, delivered to homes, assisted living communities, and nursing homes.
Elderplan has spent more than 40 years helping older adults stay healthy and independent. Aspire Healthcare Solutions shares that goal, and we help make it practical by bringing specialty care directly to Elderplan members wherever they live. Our team of experts provides mobile optometry, podiatry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. For Elderplan Medicare Advantage members, dual-eligible members, HomeFirst long-term care members, and the families and facility teams who support them, at-home specialty visits remove one of the biggest obstacles to consistent care: the trip itself.

Elderplan At-Home Care FAQs
What is Elderplan?
Elderplan is a New York not-for-profit health plan that has served older adults for more than 40 years. It is a participating agency of MJHS Health System, which allows it to connect members with additional health care resources and supportive services. Elderplan offers Medicare Advantage plans, Dual-Eligible Special Needs Plans for people with both Medicare and Medicaid, Institutional Special Needs Plans for people living in nursing homes and other congregate care settings, and HomeFirst, its managed long-term care plan. Members have access to care management, wellness programs, and benefits that vary by plan, such as transportation or over-the-counter allowances. Aspire Healthcare Solutions complements that support by bringing specialty care, including eye exams, podiatry, hearing care, and lab testing, directly to Elderplan members at home or in their care community.
What is Elderplan HomeFirst?
HomeFirst is Elderplan’s managed long-term care plan. It works with members, their doctors, and their caregivers to coordinate long-term care services such as home care, personal care, and adult day health care, along with other covered services. Managed long-term care plans in New York are designed for people who need ongoing support to live safely in the community, and many HomeFirst members already receive help at home on a regular basis. New York’s managed long-term care benefit package also includes services like podiatry, optometry and eyeglasses, and audiology and hearing aids, typically arranged through the member’s care plan. Because HomeFirst members often find travel difficult, at-home specialty visits from Aspire Healthcare Solutions can make it far easier to keep up with foot, eye, and hearing care.
Does Elderplan cover podiatry, eye exams, and hearing care?
Elderplan Medicare Advantage plans cover the medically necessary services Original Medicare covers, including a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, and diagnostic hearing and balance exams. Many Elderplan plans also include additional benefits that can apply to routine vision, eyewear, hearing, or foot care, but the details vary from plan to plan. For HomeFirst members, podiatry, optometry and eyeglasses, and audiology and hearing aids are part of New York’s managed long-term care benefit package and are usually arranged through the member’s care plan. Some services may require a referral or prior authorization. Your Evidence of Coverage, Member Handbook, or care manager can confirm exactly what is covered. Aspire Healthcare Solutions provides each of these services at home.
What is an Institutional Special Needs Plan like Elderplan Select?
An Institutional Special Needs Plan, or I-SNP, is a type of Medicare Advantage plan built for people who live in a nursing home or another institutional or congregate care setting. Elderplan offers two of these plans: Elderplan Select, for Medicare beneficiaries living in one of Elderplan’s contracted assisted living communities, nursing homes, or congregate care settings, and Elderplan Advantage for Nursing Home Residents, for Medicare or dual-eligible members living in one of Elderplan’s contracted nursing homes. These plans are designed to provide an additional level of support in the setting where the member lives. Because residents in these settings often face real challenges leaving the facility for appointments, on-site specialty care from Aspire Healthcare Solutions can help them receive eye, foot, hearing, and diagnostic care without the disruption of outside travel.
Do Elderplan HMO-POS plans require referrals?
Elderplan’s Medicare Advantage plans are HMO-POS plans, which generally coordinate care through a network of participating providers. Some HMO-POS plans require a referral from a primary care provider before seeing certain specialists, and some services require prior authorization from the plan. The point-of-service feature may allow coverage for certain services outside the network, usually at a higher cost. Because requirements vary by plan and service, the most reliable way to know what applies before an at-home eye exam, podiatry visit, or hearing evaluation is to check your Evidence of Coverage or call Elderplan Member Services. If a referral is needed, your primary care provider’s office can usually help, and Aspire’s team can schedule your visit once you are ready.
Why Specialty Care at Home Matters for Elderplan Members
Many Elderplan members live with several chronic conditions, limited mobility, or a need for daily support. Some plans include transportation benefits, and those help. But even with a ride arranged, a specialist appointment can mean getting dressed and ready early, transferring in and out of a vehicle, waiting in a busy office, and arriving home exhausted. For members who use wheelchairs, rely on aides, or live in a care facility, one visit can take an entire day and several people’s time.
That burden is why specialty care is so often postponed. A diabetic eye exam gets pushed to next year. Painful toenails go untreated. A hearing problem goes unaddressed until conversations with family become frustrating. These are not minor inconveniences for older adults. Untreated foot problems can affect walking and balance, poor vision makes daily tasks less safe, and hearing loss can lead to isolation. Bringing the specialist to the member means these needs get addressed when they should, not when travel finally becomes possible.

Supporting Elderplan Members in Nursing Homes and Assisted Living
Several Elderplan plans are designed specifically for people living in nursing homes, assisted living communities, and other congregate care settings. For these members, an outside appointment is rarely simple. It often requires scheduling transportation, assigning a staff member to accompany the resident, and planning around meals, medications, and therapy sessions.
Aspire Healthcare Solutions specializes in on-site care for long-term care facilities and assisted living communities. Our mobile optometry, podiatry, audiology, and diagnostic services come to the resident, which reduces disruption for both residents and staff. Facility teams and families can contact Aspire to discuss the services residents need, and our flexible scheduling helps visits fit into the facility’s daily routine.
How Elderplan and HomeFirst Benefits Work With At-Home Care
Most Elderplan Medicare Advantage plans are HMO-POS plans. These plans use a network of participating providers and may allow some out-of-network coverage for certain services, often at a higher cost. HMO-POS plans may require referrals for some specialty care, and certain services require prior authorization.
Every Elderplan Medicare Advantage plan covers the medically necessary services Original Medicare covers. Under Medicare Part B, that includes a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, diagnostic hearing and balance exams, and medically necessary lab tests. Many plans add extra benefits, which vary by plan.
For HomeFirst members, New York’s managed long-term care benefit package includes services such as podiatry, optometry and eyeglasses, audiology and hearing aids, durable medical equipment, and personal emergency response systems. These services are typically arranged through the member’s care plan, and many require authorization.
Your Evidence of Coverage, Member Handbook, or care manager is the best source for what your specific plan covers. Aspire Healthcare Solutions is an independent healthcare provider. We do not sell, recommend, or enroll members in health plans. Our focus is providing expert specialty care where members live.
