Hamaspik Choice Providers for At-Home Specialty Care
Expert podiatry, eye, hearing, and diagnostic care for Hamaspik Choice, Hamaspik Medicare Select, and Hamaspik Medicare Choice members, without leaving home.
Hamaspik Choice exists to help people receive long-term care at home and stay connected to their communities. Specialty care should work the same way. Aspire Healthcare Solutions brings expert specialty services directly to Hamaspik members, so foot care, eye exams, hearing evaluations, and lab work no longer depend on arranging transportation and spending a day away from home. Since 2010, our team of experts has provided mobile podiatry, optometry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. For members who already receive daily support at home, specialty care that comes to the door is simply the natural next step.

Hamaspik’s Three Plans and Where At-Home Specialty Care Fits
Hamaspik Choice (MLTC) is a managed long-term care plan that covers long-term care services so members can remain in their homes and communities. Members work with nurse care managers and social work care managers who build individualized care plans and communicate with their physicians. Because Hamaspik Choice members already receive care at home, adding at-home specialty visits keeps their care in one familiar setting.
Hamaspik Medicare Select (HMO D-SNP) is a Medicare Advantage and prescription drug plan for people who qualify for both Medicare and Medicaid. Members receive their Medicare coverage through one plan designed around the needs of dual-eligible individuals.
Hamaspik Medicare Choice (MAP) is a Medicaid Advantage Plus plan, which combines Medicare Advantage coverage with Medicaid long-term care benefits. It is built for dual-eligible members who also need long-term care services, bringing both sets of benefits together under one plan.
Whichever Hamaspik plan you or your loved one has, Aspire can bring specialty care directly to the home.
Hamaspik Choice At-Home Care FAQs
What is Hamaspik Choice?
Hamaspik Choice is a managed long-term care plan, often called an MLTC plan, that covers long-term care services so members can stay in their homes and communities. Its covered services include home health aides, personal care aides, consumer directed personal assistance, skilled nursing, adult day health care, physical, occupational, and speech therapy, home-delivered meals, and social and environmental supports. The plan also lists podiatry, optometry, audiology, dentistry, durable medical equipment, prosthetics and orthotics, and personal emergency response systems among its covered services. Members work with nurse and social work care managers who customize their care plans and communicate with their physicians. Hamaspik also offers two Medicare plans for dual-eligible members, Hamaspik Medicare Select and Hamaspik Medicare Choice. Aspire Healthcare Solutions complements this support by bringing specialty care directly to members at home.
Who qualifies for Hamaspik Choice?
According to Hamaspik, individuals must be at least 18 years old, be eligible for Medicaid, be capable of remaining safely in their home and community, live in one of the plan’s covered counties, and be eligible for certain long-term care services, such as in-home nursing, in-home therapies, home health aide services, personal care, consumer directed personal assistance, adult day health care, or private duty nursing. People who are new to Medicaid community-based long-term care services, or who have not received these services for more than 45 days, must first complete an assessment through the New York Independent Assessor to confirm eligibility. Hamaspik Member Services can answer questions about the enrollment process. Aspire Healthcare Solutions does not handle plan eligibility or enrollment, but we can bring specialty care to members once their coverage is in place.
What is the difference between Hamaspik Medicare Select and Hamaspik Medicare Choice?
Both plans are designed for people who qualify for both Medicare and Medicaid, but they serve different needs. Hamaspik Medicare Select is an HMO Dual-Eligible Special Needs Plan, or D-SNP, that provides Medicare Advantage and prescription drug coverage for dual-eligible members. Hamaspik Medicare Choice is a Medicaid Advantage Plus plan, or MAP, which combines Medicare Advantage coverage with Medicaid long-term care benefits in one plan. MAP plans are designed for dual-eligible people who also need long-term care services, so their medical care and long-term support can be coordinated together. Each plan’s Evidence of Coverage explains its specific benefits, and Hamaspik Member Services can help members understand which plan they have. Aspire Healthcare Solutions provides at-home specialty care for members of either plan.
Does Hamaspik Choice cover podiatry, eye care, and hearing care?
Hamaspik Choice lists podiatry, optometry, and audiology among its covered services, along with dentistry, durable medical equipment, and prosthetics and orthotics. These services are generally provided when medically necessary, are often arranged through the member’s care plan, and may require prior authorization. For members of Hamaspik Medicare Select or Hamaspik Medicare Choice, Medicare-covered services such as a yearly diabetic retinopathy exam for people with diabetes, foot care for diabetes-related nerve damage, and diagnostic hearing exams are also part of their coverage. The member’s care manager and Member Handbook are the most reliable sources for how each service is covered and approved. Aspire Healthcare Solutions provides mobile podiatry, optometry, and audiology at home, helping members access these services without the burden of travel.
Is Hamaspik an insurance company?
Hamaspik Choice and Hamaspik’s Medicare plans are health plans. Hamaspik Choice is a managed long-term care plan with a New York State contract, while Hamaspik Medicare Select and Hamaspik Medicare Choice are Medicare Advantage plans with Medicare contracts, and Hamaspik Medicare Choice also holds a New York State contract as a Medicaid Advantage Plus plan. These plans coordinate and cover services for their members through a network of providers. Aspire Healthcare Solutions is not an insurance company, broker, or part of Hamaspik. We are an independent healthcare provider that brings specialty care, including podiatry, eye exams, hearing care, and lab collection, directly to patients at home. For questions about Hamaspik plans or enrollment, contact Hamaspik Member Services, New York Medicaid Choice, or 1-800-MEDICARE.
Hamaspik Choice Benefits That Line Up With Aspire’s At-Home Services
Hamaspik Choice lists a wide range of covered services. Several of them correspond directly to the specialty care Aspire provides at home:
- Podiatry: Aspire’s mobile podiatry covers diabetic foot care, foot pain, and foot and ankle conditions.
- Prosthetics and orthotics: Our podiatry team provides orthotics for comfort, support, and stability.
- Optometry: Our mobile optometry team performs comprehensive and dilated eye exams, retinal imaging, vision tests, diabetic eye exams, and helps with glasses.
- Audiology: We offer at-home hearing evaluations, hearing aid fittings, and ear wax removal.
- Durable medical equipment: Aspire provides durable medical equipment to support health, mobility, and daily living.
- Personal emergency response system: We offer medical alert systems and medical alert key lock boxes for added safety at home.
Covered services under any plan are generally provided when medically necessary, often require prior authorization, and are typically arranged through the plan’s network and the member’s care plan. Your Hamaspik care manager and Member Handbook are the best sources for how a specific service is approved and covered.
Why Specialty Visits Belong at Home for Long-Term Care Members
People enrolled in a managed long-term care plan typically need help with everyday activities, which is exactly why leaving home for an appointment can be so difficult. A single podiatry or eye exam visit may require a wheelchair-accessible ride, an aide to accompany the member, careful timing around meals and medications, and a long recovery afterward. When appointments are that demanding, they tend to be delayed, and specialty needs quietly go unmet.
The consequences are real. Untreated foot problems can lead to pain, infection, and trouble with standing or transfers. Outdated glasses make reading, recognizing faces, and moving around the home harder and less safe. Hearing loss makes it difficult to communicate with aides, care managers, and loved ones. When the specialist comes to the member, these needs can be addressed promptly, comfortably, and without disrupting the support system already in place at home.
Working Alongside Your Hamaspik Care Manager
Hamaspik Choice members have care managers who coordinate their services and keep their care plans current. That coordination is a real advantage when arranging specialty care. Before scheduling an at-home visit, families can ask the care manager whether the service is part of the current care plan, whether prior authorization is needed, and whether any recent health changes should be shared with the visiting clinician.
Families and care managers can reach out to Aspire directly to discuss the services a member needs and find a visit time that fits around home care schedules. Aspire Healthcare Solutions is an independent healthcare provider. We do not sell, recommend, or enroll members in health plans. Our role is providing expert care at home.
