MetroPlusHealth Members: Specialty Care That Comes to You
Expert eye, foot, hearing, and diagnostic care for MetroPlusHealth Medicare and Medicaid members, delivered to your home or care community.
MetroPlusHealth has helped members find low-cost, quality coverage since 1985, backed by NYC Health + Hospitals, the largest municipal health system in the United States. That hospital and clinic network is a real strength. But for members who struggle to leave home, even the closest clinic can feel far away. Aspire Healthcare Solutions brings specialty care to MetroPlusHealth members right where they live. Since 2010, our team of experts has provided mobile optometry, podiatry, audiology, in-home lab collection, PAD testing, and diagnostic screenings for patients in private homes, senior apartments, assisted living communities, and long-term care facilities. When getting to an appointment is the hardest part, we make the appointment come to you.

MetroPlusHealth At-Home Care FAQs
Is MetroPlus Medicaid?
MetroPlusHealth offers Medicaid coverage, but Medicaid is only one of its plan options. The plan offers Medicaid Managed Care, along with Medicare plans, the Essential Plan, Child Health Plus, and other coverage types. Its Medicare lineup includes MetroPlus Platinum Plan (HMO) for people with Medicare, plus two plans for people with both Medicare and Medicaid: MetroPlus Advantage Plan (HMO D-SNP) and MetroPlus UltraCare (HMO D-SNP). The name and plan type on your member ID card tell you which coverage you have, and MetroPlusHealth Member Services can answer questions about your benefits. Whatever your MetroPlusHealth plan, Aspire Healthcare Solutions can bring eye care, foot care, hearing care, and lab collection to your home.
What is MetroPlus UltraCare?
MetroPlus UltraCare (HMO D-SNP) is a Medicaid Advantage Plus plan designed for people who have Medicare and full Medicaid and need long-term care services, such as home care and personal care. It combines most Medicare and Medicaid benefits in one plan, including long-term care, home and community-based services, and prescription drug coverage. Because UltraCare members typically need ongoing support to live safely at home, traveling to specialist appointments can be especially difficult. New York’s long-term care benefit package includes services such as podiatry, optometry and eyeglasses, and audiology and hearing aids, usually arranged through the member’s care team. Aspire Healthcare Solutions brings these types of specialty services directly to members at home.
Does MetroPlus Platinum cover hearing aids and glasses?
MetroPlus Platinum Plan (HMO) includes extra benefits beyond Original Medicare, and MetroPlusHealth’s 2026 plan materials list allowances for hearing aids and eyewear among them. Original Medicare generally does not cover hearing aids or routine eyeglasses, so these extra benefits can make a meaningful difference. Allowance amounts, frequency limits, and provider rules depend on the plan, so the Summary of Benefits and Evidence of Coverage are the best places to confirm your benefits. Aspire Healthcare Solutions provides at-home hearing evaluations, hearing aid fittings, eye exams, and glasses, giving MetroPlus Platinum members a convenient way to address vision and hearing needs without traveling to an office.
How do I find a doctor with MetroPlusHealth?
The most reliable way to find providers who participate in your MetroPlusHealth plan is to use the Find a Doctor tool on MetroPlusHealth’s website or call the Member Services number on your ID card. Networks can vary by plan, and provider participation can change, so it is a good idea to confirm before scheduling any appointment. MetroPlusHealth plans are HMO plans that generally use a network of participating providers, and some services may require a referral or prior authorization. If you are looking for a podiatrist, eye doctor, or hearing specialist and find it difficult to travel, Aspire Healthcare Solutions brings those services directly to your home. Call our team to discuss your needs.
Does MetroPlusHealth cover podiatry?
MetroPlusHealth Medicare plans cover the podiatry services Original Medicare covers, including foot exams and treatment for people with diabetes-related nerve damage and medically necessary care for injuries and conditions of the foot and ankle. Routine foot care for people without a qualifying medical condition is generally not covered under Original Medicare rules, although plan benefits can differ. For MetroPlus UltraCare members, podiatry is part of New York’s long-term care benefit package. Medicaid Managed Care and Essential Plan members should check their Member Handbook for podiatry coverage. Because rules vary, MetroPlusHealth Member Services can confirm what applies to your plan. Aspire Healthcare Solutions offers mobile podiatry at home, including diabetic foot care, treatment for foot pain, and orthotics.
Clinic-Based Care Is a Strength. Home-Based Specialty Care Fills the Gap.
MetroPlusHealth’s connection to a major hospital system gives members access to a wide range of care. For many members, though, the challenge is not finding a provider. It is physically getting there. Seniors who use walkers or wheelchairs, adults living with disabilities or chronic pain, and people recovering from illness often find that a single clinic visit takes an entire day and leaves them exhausted.
Specialty care is especially vulnerable to delay. Eye exams, podiatry visits, and hearing evaluations are easy to postpone because they rarely feel urgent in the moment. Over time, though, missed specialty care adds up. Diabetic eye disease can progress quietly. Foot problems can lead to pain, infection, and trouble walking. Hearing loss can make it harder to stay connected with family and to communicate with doctors. At-home visits keep these needs from slipping through the cracks.

Which MetroPlusHealth Plan Do You Have?
MetroPlusHealth offers three Medicare plans for 2026, each designed for a different type of member:
MetroPlus Platinum Plan (HMO) is for members who have Medicare Part A and Part B. It includes traditional Medicare benefits plus extra benefits from MetroPlusHealth, such as allowances that can help with eyewear and hearing aids.
MetroPlus Advantage Plan (HMO D-SNP) is a Dual-Eligible Special Needs Plan for members who have Medicare and receive assistance from New York State Medicaid. It adds extra benefits on top of Medicare coverage, including transportation to doctor visits.
MetroPlus UltraCare (HMO D-SNP) is a Medicaid Advantage Plus plan for members with Medicare and full Medicaid who need long-term care services, such as home care and personal care. It covers most Medicare and Medicaid benefits together, including long-term care and prescription drug coverage.
Every MetroPlusHealth Medicare plan covers the medically necessary services Original Medicare covers. Under Medicare Part B, that includes a yearly diabetic retinopathy exam for people with diabetes, foot exams and treatment for people with diabetes-related nerve damage, diagnostic hearing and balance exams, and medically necessary lab tests. Members with Medicaid Managed Care or the Essential Plan have their own benefits described in their Member Handbook.
Because benefits, referral rules, and prior authorization requirements vary by plan, your plan documents and MetroPlusHealth Member Services are the best sources for specifics. Aspire Healthcare Solutions is an independent healthcare provider. We do not sell insurance, recommend plans, or handle enrollment.
A Day Without the Trip
A typical specialist appointment might mean arranging a ride days in advance, getting ready early, waiting at the curb, navigating a busy building, sitting in a waiting room, completing the exam, waiting for a ride home, and spending the rest of the day recovering.
An at-home visit with Aspire means answering the door, sitting in your own chair, and receiving expert care in a familiar setting. When the visit ends, you are already home.
For members who already rely on home care or have trouble with mobility, that difference can determine whether specialty care happens at all.
